Immunization Information

Immunization Reminder:

To ensure your child is ready for the first day of school, students aged 4–6 years should be up to date with the following immunizations:

  • DTaP (Diphtheria, Tetanus, and Pertussis) – 5 doses
  • Polio – 4 doses
  • Hepatitis B – 3 doses
  • Varicella (Chickenpox) – 2 doses
  • MMR (Measles, Mumps, and Rubella) – 2 doses

You can quickly and easily access your child’s vaccination record through the Wisconsin Immunization Registry.

Important: If your child does not meet the requirements of the Wisconsin Student Immunization Law, they may be excluded from school until the necessary immunizations are documented or a valid waiver is submitted.
