WANT SPORTS? IT’S EASY AS….

 

ONLINE SPORTS REGISTRATION FOR 2025-26 IS OPEN!

This easy to use online system is designed to help make it even easier for parents to sign up their student athlete for a sport or sports during the 2025-26 school year.
Fees are still paid through Skyward Family Access or cash/check at the MHS office. You can access this new online sports registration system by clicking on the link below:

This form should be completed every other school year.  An easy way to remember this is to have it completed for a student’s Freshman and Junior years.  Physician signature required. Information about whether your student-athlete needs to have their physical examination for participation is found below.

PHYSICAL REQUIREMENTS FOR 2025-26 SCHOOL YEAR

  • BEFORE APRIL 1, 2024 – NEED PHYSICAL
  • AFTER APRIL 1, 2024-MARCH 31, 2025- GOOD FOR 1 MORE YEAR
  • APRIL 1, 2025-CURRENT DATE – GOOD FOR 2 YEARS

Parents / Guardians are able to upload physical forms directly into the registration form.
